Global Reset Week (Dec 24, 2025 - Jan 01, 2026)

In continuation of our initiative of the Global Reset Week, our team members will take a company holiday to step away and recharge. From Dec 24, 2025 to Jan 01, 2026, we’ll be operating with a reduced staff. IF-AT orders placed after Dec 11, 2025 will be confirmed and processed from Jan 06, 2026. Order deliveries will resume between Jan 07 and 15, 2026. IF-AT queries sent after Dec 22, 2025 will be attended to from 06 Jan, 2026.

IF-AT Forms

The IF-AT forms can currently be ordered in a length of 10, 25, or 50 questions with four or five answer choices.

The forms come in bundles of 125 and are available for order in multiples of 500. A total of four answer keys will be provided per order, per variation, regardless of the ordered quantity. The answer keys are updated approximately every six to nine months.

10-Question Form

25/50-Question Form

  • Available with 4 or 5 choices
  • Form can be used as a 50-question or a 25-question form
  • A perforation line between the 25th and 26th questions allows the form to be torn into two 25-question forms
  • Achieve up to 8 answer keys if used as 25-question form
  • Achieve up to 4 answer keys when the form is used on its own as a 50-question form
  • Achieve up to 28 answer keys if you choose to tear the form into two-25-question forms and pair two different 25-question forms to form a 50-question form
  • Store forms away from the sun to prevent the curing of the silver foil
  • Curing will make scratching difficult
  • Good for use within 12 months of purchase
